Core principles (site-wide)

Treat the cause first, then choose the dressing: offload the diabetic foot, compress the venous leg, revascularize the arterial limb, redistribute pressure for pressure injuries — the dressing is a supporting actor.
Judge healability first (WBP 2021): classify wounds as healable, maintenance, or non-healable; never aggressively debride dry gangrene before perfusion is assessed.
Standardize assessment & documentation: TIMERS for clinical reasoning, MEASURE as the record backbone, plus etiology-specific classifications (SINBAD/WIfI/NPIAP/CEAP) — the same wound stays comparable across dates and caregivers.
Do not give antibiotics to uninfected ulcers (IWGDF/IDSA 2023); infection is a clinical diagnosis — scales and cultures only assist.
Offloading cannot be replaced by any dressing (IWGDF Offloading 2023).
Guidelines recommend dressing "categories", not brands; evidence hierarchy: society guidelines → systematic reviews → local formulary → manufacturer IFU.
Advanced therapies (NPWT / oxygen / cellular-matrix products) only after standard care is optimized (IWGDF 2023).
Red-flag signs go to medical care first: uncontrolled bleeding, rapidly spreading redness, fever, necrosis with foul odor, deep-tissue exposure — triage before wound care.
